in the defence of Maradon, when the Trollocs were breaking through, everyone was setting up for defense, but then suddenly a horn was blown in the signal for retreat, while others were not. this almost got everyone killed.

The simple answer is that someone freaked out after seeing the trollocs closing in and blew the horn. You know its easier to "officially" retreat than to run off scared shite-less. I have never been in battle but I can imagine that chaos rules rather quickly after a military's defense is broken as the following scene in the book showed.

I prefer the idea that it was a simple human mistake. It shows that even the best General is still vulnerable to the vagaries of the battlefield.
