In the wiki it said that there were some fan theories that Birgitte being ripped out had led to Cain being spun out but it did not elaborate on what they were. I seem to remember that Birgitte said to Nyn shortly before her removal that she had not seen Cain for some time which is strange as they had been meeting regularly in what was a small amount of time.

It’s stated that GC is always significantly older than Birgitte.  When she is ripped out, this is not a normal life as she appears as an adult with an understanding of who she is and remembers some of her past lives. She dies and will then be able to be born normally, although presumably not for years at GC is only a baby at the end of the series.

I am fairly sure she is being reborn at the conclusion of the Last Battle her final discussion with Elayne says as much I think.

 

 After the battle she says farewell to Elayne, feeling that her rebirth to flesh is imminent and looking forward to finding Gaidal again

People also forget Dem was partially insane by this point, Moghi mentioned how he had changed since being around the Sharans.  He had been using the True Power a lot and one of the side effects is insanity.    He couldn't use the male half around them, so he had to rely on the True Power. Suffering from partial insanity would explain some of his behavior at the end.

Good point about the true power. And I agree, Demandred is pretty much out of his mind the whole time, so we can't apply logic to the duels.

From what I could find the quote was something along the lines of:

Yes, I know it wasn't with trumpets and fanfare [referring to Sammael's death]; but he deserved it. He was a louse, and he got a louse's death. He was killed by an enemy he wasn't paying attention to. He lived like a louse, and he died like a louse."

Sadly, most of the Forsaken were similar but got more effective deaths. Sammael was a primary antagonist far longer than Demandred and was more effective. He manipulated the Shaido, forced Graendal to act outside of her own interest and was a serious danger to Moriaine and Perrin during their travels to Tear.
