We had to make this decision for the first time right before Christmas with our oldest kitty boy (exactly one year and a morning after his brother passed away). There was really no question of treatment (congestive heart failure with lung damage from liquid, and diseased lungs that were healthy at his last bloodwork, he was never gone to be off a respirator again), and it was still the hardest thing I have ever done. (Most of our babies have, fortunately, died of old age in their own time). My son did not go with us (his choice. He said he said goodbye when we took him to the vet the night before because the cat was clearly in severe distress). 

All I can say, is I appreciate that our local University's vet med dept has a very nicely appointed (and very soundproof) good-bye room.
