Please give me a summary of the end of Book 9

What I'm looking for is a breakdown of which forsaken showed up, and what happened with them.  I remember Osan'gar (Asmodean) getting killed, but I can't remember what happened to Cyndane (Lanfear), Demandred or Graendal.  I know someone out there probably knows off the top of their head.

 

Also, a good description of how Rand cleansed the true source would be good.  I remember him gathering it into a giant black ball, and I think that ball wiped out Shadar Logoth.
